The Claims Administrator and the Parties have adopted criteria for the submission of claims seeking Extraordinary Injury Payments. The period to submit Extraordinary Injury Claims began on March 2, 2009 and ended on September 1, 2009.
DEADLINE WARNING: All EI Claim Forms and supporting Required Documentation must have been submitted to the Claims Administrator no later than September 1, 2009.
Primary Counsel for Claimants should have submitted EI Claims on line, using their Vioxx Portal. An unrepresented Claimant should have contacted the Claims Administrator to obtain an EI Claim Form for Unrepresented Claimants. |
